Learning From Experience (LFE) Teams bring workers, engineers, specialists and planners together to discuss how work really gets done, not how managements thinks it gets done. The conversations are open and honest so are able to get a much clearer picture of challenges workers encounter that lead to an unwanted event. By recognizing where existing defenses and processes are not be as robust as believed and getting workers involved in identifying better defenses to reduce the potential for errors or their consequences, we also achieve a higher degree of worker engagement and ownership.
