There was an unexpected sudden and very large process piping rupture and fire on the evening of Sunday, May 17, 2009. The cause of the piping rupture was a localized area of external corrosion at the bottom of the pipe at a pipe support contact location. This presentation will share the causes of the localized external corrosion, an understanding of improvements to piping inspection practices that are needed to detect this type of deterioration in the future, and additional lessons learned.
