Many SIS designs suffer from a disconnect between the risk analysts and the SIS designers. This disconnect results in incomplete designs and implementation of SIS in situations where thay are nor appropriate. Furthermore, subsequent phases in the SIS lifecycle are poorly designed. This paper looks at what is required from the risk analysis team from a designer and maintainer perspective. It gives guidance on what information should be contained in a LOPA along with what is typically missing.
